Occupational Therapy Degrees

Entry-Level vs Post-Professional OTD

Two kinds of Doctor of Occupational Therapy degree with different purposes, and how to tell which one a program is.

Last reviewed 3 November 2026

Key points

  • “OTD” covers more than one kind of program. An entry-level OTD prepares new students for the profession; a post-professional OTD is for people who already hold credentials.
  • Only an ACOTE-accredited entry-level program leads to eligibility for the NBCOT exam as a first route.
  • Not all doctorates are entry-level professional programs.
  • A post-professional degree does not replace NBCOT certification or a state license.

The two kinds

Entry-level OTD. This is a doctoral program for people who are not yet occupational therapists. Graduates of ACOTE-accredited entry-level programs are the ones NBCOT recognises for the OTR exam. ACOTE’s school directory lists OT doctorate programs it accredits as a category. Browse them on our entry-level OTD page.

Post-professional OTD. This is for practitioners who already completed an OT education and want advanced study, for example in leadership, education, advocacy or advanced clinical practice. It follows an earlier OT degree; it does not start the profession. Browse them on our post-professional OTD page.

The words “doctor” and “OTD” appear in both, so read the admission requirements. If a program asks for a prior OT degree or an active credential, it is post-professional. If it asks for a bachelor’s degree plus prerequisites and takes new students into the profession, it is entry-level.

Why the distinction matters

NBCOT states that U.S.-educated applicants must have graduated from an ACOTE-accredited program with an entry-level degree in occupational therapy. A post-professional degree is not described there as a substitute. Enrolling in the wrong kind of program could mean spending time and money without becoming eligible for the exam.

Remember the basic limits:

  • A degree alone does not grant a license. Licensure is set by each state; check the state board.
  • Admission does not guarantee that you will be eligible for NBCOT certification. Eligibility depends on graduating from an accredited program and meeting NBCOT’s requirements.
  • A doctorate is not universally required. ACOTE’s public guidance states that there is no mandate regarding OT doctoral-degree-level education; schools may offer a master’s, a doctorate or both. See master’s vs OTD.

How to check which kind it is

  1. Look at the stated purpose. Does the program say it prepares you for “entry into practice” or “for certification as an OT”? Or does it say it advances the practice of licensed therapists?
  2. Look at admissions. A required prior OT degree points to post-professional.
  3. Look at accreditation. In ACOTE’s directory, search the school and see whether it appears under OT Doctorate. If a post-professional program is not on that list, that is not a warning in itself, because the list covers programs that lead to entry into the profession; it only means the program cannot be your route to eligibility.
  4. Ask the program directly. Ask: “If I enrol, will I be eligible to apply for the NBCOT exam as a new graduate?”
  5. Check the state. Ask the state board whether it accepts graduates of the program for licensure.

Who chooses a post-professional OTD

People who already hold the OTR credential, or who earned an OT degree elsewhere, sometimes choose a post-professional program for career development. It may or may not affect pay or promotion; employers differ and we do not publish figures. Internationally educated applicants should read NBCOT’s separate eligibility page, which describes the process for degrees earned abroad; it should not be assumed that a post-professional degree resolves eligibility.

Delivery and fieldwork

Post-professional programs are often delivered online or part time because the students are working. Entry-level programs include supervised fieldwork that takes place in person. A program that is fully online and also claims to be entry-level deserves a close look at how fieldwork and labs are handled. See online coursework and in-person fieldwork and the online page.

What this directory records

We list entry-level and post-professional programs on separate pages and record the exact award a school publishes. See methodology. If you see an error, tell us through the corrections page.
