Occupational Therapy Degrees

OTD vs PhD in Occupational Therapy

Why a PhD is not an OTD, what each degree is for, and which one can lead to certification as an occupational therapist.

Key points

  • A PhD is a research doctorate. An OTD is a professional doctorate in occupational therapy. They are different degrees.
  • A PhD does not qualify you as an occupational therapist, and an NBCOT-eligible route runs through an ACOTE-accredited entry-level program.
  • An OTD may be entry-level or post-professional; check which.
  • Neither doctorate is required to practise; the entry-level degree is a master’s or a doctorate.

Two kinds of doctorate

OTD. A professional doctorate named for the profession. Entry-level OTD programs prepare students to enter the occupational therapy profession and are accredited by ACOTE, which lists OT doctorate programs as one category in its school directory. Post-professional OTDs are for practitioners who are already credentialed. See entry-level vs post-professional OTD.

PhD. A research degree. A PhD student designs and carries out original research, typically leading to a dissertation. PhD programs in this field may be titled occupational therapy, rehabilitation science, occupational science, health sciences or similar, depending on the university. The exact title varies, so read the program page. Browse our PhD page.

What each is for

  • Entry-level OTD: becoming an occupational therapist, in the same way a master’s degree does. Graduates of ACOTE-accredited entry-level programs can apply for the NBCOT exam.
  • Post-professional OTD: advancing practice, leadership, education or advocacy for someone already in the field.
  • PhD: becoming a researcher, often with a university faculty or research career in mind.

These labels describe typical aims. Programs publish their own goals and you should read them.

Can a PhD lead to certification?

Not on its own. NBCOT states that U.S.-educated applicants must have graduated from an ACOTE-accredited program with an entry-level OT degree (OT for the OTR credential). A PhD in a related field is not described there as an entry-level OT degree. Some people hold a clinical OT degree first and then do a PhD; the PhD is then an addition to the clinical credential, not a replacement. NBCOT’s separate process for internationally educated applicants applies only to educational credentials earned outside the United States.

Also remember that a degree alone does not grant a license. Each state sets its own requirements; see NBCOT certification and state licensure.

Do you need a doctorate at all?

No doctorate is universally required for clinical practice. ACOTE’s public guidance states that there is no mandate regarding OT doctoral-degree-level education, and that schools may offer a master’s, a doctorate or both. See master’s vs OTD. A doctorate may matter more if you want to teach at a university; university requirements differ, so check the job postings and the institution rather than relying on a general rule. We do not state how many positions require which degree.

Funding varies widely between programs, so treat the funding row as a prompt to ask, not as a fact about any school.

Questions to ask

  1. Which degree does the program award, in exact words?
  2. Does it admit people without an OT degree? If so, is it ACOTE-accredited and entry-level?
  3. If it is a PhD, what research areas and advisers are available, and how is it funded?
  4. Will I be eligible for NBCOT certification after finishing? (For a PhD the usual answer is that eligibility comes from an earlier entry-level degree.)
